Discovering prescription patterns in pediatric acute-onset neuropsychiatric syndrome patients

Pediatric acute-onset neuropsychiatric syndrome (PANS) is a complex characterized by an abrupt onset of obsessive-compulsive symptoms and/or severe eating restrictions, along with at least two concomitant debilitating cognitive, behavioral, or neurological symptoms. A wide range pharmacological interventions behavioral and environmental modifications, psychotherapies have been adopted to treat underlying etiologies. Our goal was develop data-driven approach identify treatment patterns in this cohort. In cohort study, we extracted medical prescription histories from electronic health records. We developed modified dynamic programming perform global alignment those medication histories. unique since it considers time gaps as part the similarity strategy. This study included 43 consecutive new-onset pre-pubertal patients who had 3 clinic visits. algorithm identified six clusters distinct usage history which may represent clinician’s practice treating PANS different severities etiologies i.e., most groups requiring high dose intravenous steroids; arthritic inflammatory prolonged nonsteroidal anti-inflammatory drug (NSAID); mild relapsing/remitting group treated short course NSAID. The psychometric scores outcomes each cluster generally improved within first years. shows potential improve our knowledge cohort, while helping clinicians understand how respond combination drugs.
